Charting Requirements
Some workplaces require you to complete charting (patient documentation) during or at the end of your shift. If charting is not completed, the workplace may report it, which can result in a notice on your account.
Common charting systems:
- Workplace-specific electronic health record (EHR) systems (e.g., PointClickCare, MatrixCare, Netsmart)
- Paper charting at some facilities
- Workplaces provide login credentials and instructions when you arrive
"Incomplete Charting" Notice
If you receive a notice about incomplete charting:
What to do:
- Check the notice details in your in-app notifications — it will show the shift date and workplace
- If you completed charting, contact the Support Team with: the shift date, workplace name, and confirmation that charting was completed (e.g., screenshot of the EHR submission, workplace supervisor confirmation)
- If you didn't complete charting, explain the reason to the Support Team — technical issues (system down, no login provided) may qualify for a review
Facility Login Issues
No login credentials provided:
- Ask the charge nurse or shift supervisor for login credentials when you arrive
- If no one is available to provide credentials, document the time you asked and who you spoke to
- Contact the Support Team with this information if you receive an incomplete charting notice
Login credentials not working:
- Double-check the username and password — EHR systems are often case-sensitive
- Ask the facility staff to reset the credentials or provide a temporary login
- If the system is down or no one can help, note the time and the issue, and inform the charge nurse
- Contact the Support Team if the inability to log in affected your shift documentation
EHR system down or unavailable:
- Inform the charge nurse or supervisor immediately
- Ask if paper charting is available as a backup
- Document the time the system went down and when (if) it came back
- If you receive an incomplete charting notice due to a system outage, contact the Support Team with the details
Impact of Incomplete Charting
Incomplete charting notices are tracked on the account. Depending on the workplace and frequency:
- The workplace may issue a Do Not Return (DNR)
- Repeated incomplete charting across multiple workplaces may affect account standing
- Clipboard does not adjudicate charting disputes — the workplace determines whether charting was completed
If you believe the notice is incorrect:
- Contact the Support Team
- Provide: the shift date, workplace name, and evidence that charting was completed (EHR screenshot, supervisor confirmation, paper records)
- The Support Team can relay the information to the workplace for review
Best Practices for Charting
- Ask for login credentials and charting instructions at the start of every shift — don't wait until the end
- Complete charting before leaving the workplace whenever possible
- If charting systems are unfamiliar, ask the charge nurse for a quick orientation
- Take a screenshot of your completed charting submission as a personal record
- If you need to leave before charting is complete, inform the supervisor and document how much was done
